Description

Running along the anterior compartment of the leg, the tibialis anterior tendon descends from the muscle belly on the lateral surface of the tibia and crosses the anterior aspect of the talocrural joint on its way toward the medial foot. A fibrous tendon sheath, rendered as a sleeve or tunnel, encloses the tendon as it passes deep to the extensor retinacula, with the tibia positioned medially and the fibula lateral in the same field. Distal to the ankle, the tarsals, metatarsals, and phalanges align in anatomical position, clarifying the tendon’s oblique course toward its insertion on the medial cuneiform and base of the first metatarsal. Bony landmarks matter. Focusing on the tibialis anterior tendon sheath at the front of the ankle is clinically specific because this is where friction, thickening, or stenosing tenosynovitis can produce pain and snapping during dorsiflexion and inversion, and where the tendon is vulnerable in anterior ankle lacerations. The relationship to the extensor retinaculum is also surgical, it guides longitudinal incision planning for tendon exploration or sheath release while helping you avoid the anterior tibial artery and deep fibular (peroneal) nerve that travel in the same compartment. The anterior view makes the retinacular tunnel intuitive.
