Description
Anterior thoracic anatomy centers on the paired tenth ribs (costae X) as they sweep inferolaterally from the thoracic vertebrae toward the anterolateral chest wall. Each rib’s bony shaft transitions to costal cartilage near the costochondral junction, and the tenth costal cartilage typically joins the cartilage of rib nine to form the costal margin rather than reaching the sternum. Costal grooves run along the inferior internal border of each rib, and the intercostal spaces between ribs nine to ten and ten to eleven frame where the neurovascular bundles course. These are false ribs. Attention to rib ten matters because it sits at the interface of the lower thorax and upper abdomen, where trauma, procedural access, and referred pain commonly overlap. The tenth rib is a frequent site of fracture with blows to the lateral chest, and displacement can endanger the intercostal vessels or irritate the diaphragm, producing pain that is sharp with respiration and sometimes perceived near the upper abdomen. Clinically, rib ten is also a landmark level for lower intercostal nerve blocks and for planning safe chest drain insertion zones, keeping instruments above the rib to avoid the costal groove and its bundled artery, vein, and nerve.
