Tibialis Anterior Under the Skin, Anterior View

Description

Running along the anterolateral border of the tibia, the tibialis anterior forms a broad muscle belly in the proximal leg that tapers distally into a tendon crossing the anterior ankle beneath the extensor retinacula. Medial to it sits the subcutaneous tibial crest (shinbone), while the fibula lies lateral and slightly posterior, framing the anterior compartment. Distally, the tibialis anterior tendon angles toward the medial foot to reach the medial cuneiform and base of the first metatarsal, with neighboring extensor hallucis longus and extensor digitorum longus tendons occupying a more central and lateral position on the dorsum. Skin and superficial fascia overlay the compartment, keeping the surface topography clinically recognizable. Anterior leg anatomy matters when dorsiflexion fails. Weakness or denervation from deep fibular (peroneal) nerve injury produces foot drop and a compensatory high-stepping gait, and this view helps connect that deficit to the anterior compartment muscles and their tendons crossing the ankle joint. Tightness or swelling within the anterior compartment can drive acute compartment syndrome after tibial fracture or exertional overuse, and the relationship of tibialis anterior to the tibial crest and intermuscular septa guides safe fasciotomy placement. Tendinopathy at the anterior ankle is common in runners and hikers. Palpation localizes it.
