Description
Centered at the end of the vaginal canal, the ectocervix is shown encircled by the vaginal fornices, with the external os sitting centrally and leading into the endocervical canal toward the uterine cavity. Surrounding squamous epithelium appears smoother and paler, while the area labeled cervical erosion corresponds to exposed columnar epithelium (cervical ectropion) on the portio vaginalis, positioned around the os and extending onto the anterior and posterior cervical lips. Fine branching vessels and a moist surface emphasize the glandular mucosa, which lies more proximal and superior toward the uterus than the distal vaginal walls. Orientation matches a speculum examination with the cervix viewed directly along the vaginal axis. Clear landmarks. Cervical erosion matters because it is a frequent benign finding that can mimic pathology during screening and colposcopy, and it often explains contact bleeding after intercourse or a persistent mucoid discharge without implying malignancy. Glandular epithelium on the ectocervix is friable and acid-sensitive, so acetic acid application during colposcopy can accentuate the transformation zone and help distinguish ectropion from cervicitis, true ulceration, or high-grade squamous intraepithelial lesions near the squamocolumnar junction. Clinicians also associate ectropion with pregnancy, adolescence, and estrogen exposure (including combined oral contraceptives), contexts that shift the squamocolumnar junction outward onto the ectocervix. Use this illustration for teaching normal versus abnormal speculum findings in obstetrics and gynecology modules, for explaining Pap test sampling of the transformation zone, and for correlating patient symptoms with a benign cervical examination in clinic handouts and patient counseling materials.
