Description
Sagittal sectioning through the female pelvis brings the cervix uteri into profile, separating its vaginal part (portio vaginalis cervicis) inferiorly from the supravaginal part superiorly as it continues into the uterine body at the internal os. The cervical canal runs obliquely between the external os opening into the vaginal lumen and the internal os at the uterine cavity, with the anterior cervical wall related to the urinary bladder and the posterior wall facing the rectouterine space (pouch of Douglas). Superior to the cervix, the isthmus and lower uterine segment sit centrally, while the vagina surrounds the ectocervix inferiorly. Clear landmarks. This plane matters because many clinical decisions hinge on cervical length and the relationship of the internal os to the lower uterine segment, the core measurement in transvaginal ultrasound assessment for risk of preterm birth and cervical insufficiency. It also maps directly onto pathways for instrumentation, including dilation and curettage and hysteroscopy, where the angulation between the cervical canal and uterine cavity influences ease of entry and risk of creating a false passage or perforation. The cervix is not a straight tube.
