Description
Sagittal division through the uterus exposes the uterine wall in profile, with the myometrium forming the thick muscular coat between the endometrium lining the uterine cavity and the outer serosal surface (perimetrium). Distinct inner, middle, and outer myometrial layers are differentiated by fiber orientation, with the stratum subvasculare adjacent to the endometrium, the thicker stratum vasculare centrally, and the outer stratum supravasculare blending toward the uterine surface. Superiorly the wall thickens toward the fundus, while inferiorly it tapers toward the isthmus and cervical region. Orientation is clean and didactic. Reading the myometrium by layers matters when you are teaching uterine contractility and hemostasis. The middle vascular layer, with its circumferential and interlacing bundles around the arcuate and radial branches, is the mechanical basis for postpartum uterine compression of vessels, and loss of tone here underlies uterine atony and postpartum hemorrhage. Same anatomy, different clinic: adenomyosis represents ectopic endometrial glands and stroma within the myometrium, often centered in the junctional zone, a concept that becomes clearer when the wall is seen in sagittal section.
