Description
Viewed from an inferior perspective within the infratemporal fossa, the lateral pterygoid muscle is seen deep to the mandibular ramus, with its superior (upper) head arising from the infratemporal surface and crest of the greater wing of the sphenoid and its inferior (lower) head arising from the lateral surface of the lateral pterygoid plate. Both heads course posterolaterally and superiorly toward the temporomandibular joint, converging onto the pterygoid fovea on the anterior neck of the mandible and the capsule and articular disc of the TMJ. Medial to it lie the medial pterygoid and the pterygoid plate region, while the mandibular condyle sits posterolateral as the primary bony landmark in this inferior view. An inferior look at the lateral pterygoid matters because it is the only muscle of mastication that inserts into the TMJ disc and capsule, so its fiber direction explains anterior translation of the condyle during mouth opening and contralateral excursion during chewing. Disc displacement with reduction, a common TMJ disorder, is often discussed in terms of altered disc-condyle coordination, and this angle makes it easier to teach where the superior head can influence disc position during closing and clenching. Botulinum toxin injection and surgical work around the condylar neck also demand respect for this deep, confined corridor. Tight anatomy.
