Description
Inferiorly, the visceral surface of the liver is oriented toward the viewer, with the porta hepatis occupying a transverse cleft on the undersurface where the portal vein enters and the hepatic artery proper and bile ducts course in close apposition within the hepatoduodenal ligament. Tributary venules radiate through the parenchyma toward larger hepatic venous channels, which converge superiorly and posteriorly to drain into the inferior vena cava in the caval groove. Medially, the fissure for the ligamentum teres and the adjacent fissure for the ligamentum venosum mark the left sagittal fissure, while the quadrate and caudate lobes bracket the porta hepatis anteriorly and posteriorly. Clinically, an inferior perspective is the working viewpoint for hepatobiliary surgery and portal triad control. The relationships at the porta hepatis define the Pringle maneuver, and they also explain why portal hypertension from cirrhosis redirects flow into portosystemic collaterals, producing esophageal varices and recanalization of paraumbilical veins. Hepatic venous outflow matters too, because obstruction at the hepatic veins or their junction with the IVC (Budd-Chiari syndrome) produces congestive hepatomegaly that can be correlated with the directionality of venous drainage. Use this illustration for teaching abdominal visceral anatomy in gross anatomy and surgical anatomy courses, and for publishing clear schematics on portal venous inflow versus hepatic venous outflow in hepatology, radiology (correlation with Doppler ultrasound and contrast CT portography), and operative technique manuals.
