Description
Arising from the posterior aspect of the liver, the right, middle, and left hepatic veins converge toward the inferior vena cava as they exit the parenchyma near the bare area. Thick-walled, blue venous trunks course superiorly and posteriorly, while progressively smaller venules radiate peripherally within the lobular tissue, giving a clear proximal-to-distal sense of hepatic drainage. The vena cava lies immediately posterior to the hepatic tissue, with the largest-caliber venous segments clustered closest to the caval entry points. Surface lobulation and the smooth capsular contour help orient the organ’s superior and inferior margins. Venous outflow anatomy is where liver surgery gets unforgiving. Precise understanding of hepatic vein confluence patterns and their short extrahepatic segments at the inferior vena cava underpins safe right or left hepatectomy, living-donor graft procurement, and control of bleeding during parenchymal transection.
