Description
Oriented in a left lateral view of the male heart, the pulmonary veins course from the posterior aspect of the left lung toward the posterior wall of the left atrium, entering superior to the atrioventricular (mitral) valve plane. From this perspective the left superior and left inferior pulmonary veins are seen converging medially, while the left atrial appendage projects anteriorly and slightly superiorly along the atrioventricular groove. The pulmonary trunk and left pulmonary artery lie anterior to the venous confluence, and the aortic arch sweeps superiorly, with coronary arteries and cardiac veins tracing across the epicardial surface. Pulmonary venous anatomy is a frequent source of clinical and procedural complexity because the venous ostia and the posterior left atrial wall form the substrate targeted in atrial fibrillation ablation, where isolation lines are placed around the pulmonary vein antra. A left-sided view clarifies why the pulmonary veins, although carrying oxygenated blood, remain posterior structures relative to the pulmonary arteries and great vessel outflow, a relationship that matters during transseptal catheter navigation and when interpreting CT or MR angiography of the left atrium. Small variants, such as a common left pulmonary venous trunk, also become easier to conceptualize when you start from the lateral approach.
