Description

Seen from the anterior thorax, the right superior pulmonary vein and right inferior pulmonary vein course medially from the hilum of the right lung toward the left atrium, with segmental venules converging from the upper and lower lobes. A separate right middle pulmonary venous channel is suggested by drainage from the middle lobe, positioned inferior to the superior vein and superior to the inferior vein as it approaches the atrial inflow. Behind the translucent lung parenchyma, the trachea bifurcates into the right main bronchus, which runs posterior to the pulmonary vessels at the root of the lung. The cardiac silhouette sits medial and slightly inferior to the venous convergence. Anterior orientation matters because right pulmonary venous anatomy drives procedural safety at the left atrium. Variants such as a distinct right middle pulmonary vein or a common right pulmonary venous ostium change ablation strategy for atrial fibrillation and help explain post-procedural pulmonary vein stenosis when energy delivery encroaches on the venous wall.
