Description
Arising from the superior mental spines (genial tubercles) on the inner surface of the mandible, the paired genioglossus muscles are shown from an inferior perspective as broad, fanned sheets that diverge posteriorly and superiorly into the substance of the tongue. Medially, the right and left muscles approximate near the lingual septum, while their more lateral fibers spread toward the margins of the tongue, sitting superior to the floor of the mouth and anterior to the oropharyngeal inlet. Superiorly in the field, the maxillary dentition and hard palate frame the oral cavity, and posteriorly the skull base transitions toward the upper cervical spine with the cervical vertebral bodies and intervertebral discs aligned in the midline. Fiber direction matters here. The genioglossus is the principal protrusor of the tongue and a key anterior stabilizer of the tongue base, so its mandibular origin and fanlike insertion pattern explain why loss of tone during sleep predisposes the tongue to fall posteriorly and contribute to obstructive sleep apnea. This angle also maps cleanly onto airway management and surgical planning: the relationship of the tongue base to the palate and pharyngeal space is the same corridor assessed during laryngoscopy, and it is the target region for procedures such as genioglossus advancement for hypopharyngeal collapse. A reliable landmark.
