Description
Posteriorly, the right and left lobes of the glandula thyroidea flank the upper trachea and cricoid cartilage, joined across the midline by the isthmus at a slightly inferior level. The posterior contour places the medial surfaces close to the laryngotracheal groove, while the lateral aspects approach the carotid sheath region. Along the posteromedial margin, the tracheoesophageal groove and anterior surface of the esophagus sit immediately deep to the thyroid, and the superior poles rise toward the level of the thyroid cartilage. This posterior perspective matters because it aligns with the operative anatomy that drives complication profiles in thyroidectomy. The recurrent laryngeal nerves typically ascend in the tracheoesophageal grooves and cross near the ligament of Berry as the gland anchors to the cricoid and upper trachea, a tight plane where traction or thermal injury can occur. Parathyroid glands also sit on the posterior aspect (commonly superior parathyroids near the cricoid level and inferior parathyroids more variable), making the posterior contour the teaching view for preserving parathyroid vascular supply and avoiding postoperative hypocalcemia.
