Description

Posteriorly oriented neck anatomy centers on the larynx and proximal trachea, with the right and left lobes of the thyroid gland (glandula thyroidea) flanking the airway and meeting across the midline via the isthmus at the lower cervical level. Against the posterior surface of each thyroid lobe sit the small, ovoid parathyroid glands, classically a superior pair nearer the upper pole and an inferior pair closer to the lower pole, positioned medial to the carotid sheath and lateral to the laryngotracheal complex. Superiorly, the laryngeal framework is visible, including the epiglottis and adjacent cartilages at the hypopharyngeal level, while inferiorly the trachea (windpipe) continues caudally with its cartilaginous rings defining the anterior and lateral tracheal wall. A posterior view matters because it mirrors the surgical reality of thyroidectomy and parathyroid exploration, where the parathyroid glands are sought on the dorsomedial aspect of the thyroid rather than on its anterior surface. Their variable position, often near the terminal branches of the inferior thyroid artery, explains why devascularization can lead to postoperative hypocalcemia, even when the glands are left in situ. It also reinforces the spatial logic of airway protection and voice production, with the larynx superior to the trachea and closely related to the posterior pharyngeal wall.
