Description

Anteriorly, the right lung is presented with emphasis on the middle lobe and its lateral (outer) bronchopulmonary segment, Segmentum laterale, distinguished by color. The segment occupies the anterolateral portion of the right hemithorax, bordered superiorly by the horizontal fissure separating it from the upper lobe and inferiorly by the oblique fissure separating it from the lower lobe. Medially, its parenchyma relates to the middle lobe hilum where the segmental (tertiary) bronchus and accompanying pulmonary arterial branch enter, while the visceral pleura defines the peripheral contour laterally. Fissures and pleural margins are the key surface landmarks. Segmental anatomy matters most when you need to predict where disease will localize and how it will spread. The lateral segment of the right middle lobe is a common site of recurrent atelectasis and infection in right middle lobe syndrome, often related to impaired drainage or obstruction of the middle lobe bronchus, and an anterior view helps correlate segmental boundaries with bronchoscopy findings and lobar collapse patterns on chest radiography. Radiologists and surgeons also rely on these borders when planning a segmentectomy, where segmental veins typically course in the intersegmental planes and guide parenchymal division. Pulmonary and thoracic surgery teaching files can pair this illustration with bronchoscopy anatomy to reinforce the hierarchy from lobar to segmental bronchi. It also fits well in anatomy lab manuals, respiratory modules, and clinical radiology atlases that need a clean reference for right middle lobe segmentation and fissural relationships in anterior projection.
