Description
Arising from the right main bronchus just distal to the carina, the right intermediate bronchus is presented in anterior orientation as a short airway segment between the origin of the right superior lobar bronchus superiorly and the bifurcation into the middle and inferior lobar bronchi inferiorly. From this perspective, the intermediate bronchus courses inferolaterally toward the right hilum, with its lumen directed posterior to the great vessels that cross the mediastinum. The branching geometry, short length, and slight rightward angulation are emphasized. A transitional segment by design. Clinically, the right intermediate bronchus is a common site for mucus plugging, endobronchial tumors, and foreign body impaction because the right-sided bronchial tree tends to align more directly with the trachea than the left. For bronchoscopy, this segment functions as the gateway to both the right middle lobe bronchus and the right lower lobe bronchus, so subtle narrowing or extrinsic compression can present as combined middle and lower lobe atelectasis or recurrent post-obstructive pneumonia. Its proximity to the right pulmonary artery and hilar lymph node stations also makes it a frequent focus in lung cancer staging and when interpreting airway indentation on CT.
