Description
Arising from the right lower lobe bronchus, the medial basal segmental bronchus (B7) courses inferiorly and medially toward the mediobasal parenchyma of the right lung. From an anterior perspective, the right main bronchus is seen descending from the trachea, giving rise to the upper lobe bronchus and bronchus intermedius, then continuing to the lower lobe where the basal segmental bronchi branch in a fan-like pattern. The medial basal branch sits medial to the anterior basal segmental bronchus (B8) and typically lies closer to the mediastinal surface than the lateral and posterior basal bronchi (B9, B10). Orientation is clear. Segmental bronchial anatomy matters when you are planning or interpreting a segmentectomy, because the boundaries of resection follow segmental bronchi and their accompanying pulmonary arteries more reliably than surface fissures. B7 is also a frequent source of nomenclature errors, since the medial basal segment may be small, absent, or share a common trunk with B8 (the so-called anteromedial basal bronchus) on bronchoscopy and CT. An anterior airway map like this helps reconcile what the bronchoscopist sees with right lower lobe segmental labeling used in thoracic surgery and radiology reports.
