Description
Arising as the narrowed isthmus between the pancreatic head and body, the neck (collum pancreatis) lies anterior to the superior mesenteric vessels and inferior to the pylorus, with the first part of the duodenum curving to its right. From an anterior perspective, the head remains to the subject’s right and cradled by the duodenum, while the body extends leftward toward the splenic hilum, leaving the neck as the transitional zone between these larger segments. The main pancreatic duct typically courses within the gland toward the neck before continuing into the body. A tight corridor of anatomy. Surgeons and radiologists focus on the neck because it overlies the confluence of the superior mesenteric vein with the splenic vein to form the portal vein, a relationship that drives resectability decisions in pancreatic adenocarcinoma and dictates where tumor encasement or venous abutment is assessed on contrast-enhanced CT. During pancreaticoduodenectomy, transection commonly occurs at the neck over the portal-supramesenteric venous axis, where small venous branches can make bleeding control unforgiving. Pancreatitis-related inflammation in this region can also tether adjacent vessels and complicate dissection.
