Description
Crista galli rises in the midline as a sharp, superiorly directed ethmoidal crest, positioned just posterior to the frontal bone and continuous inferiorly with the perpendicular plate of the ethmoid. From an anterior perspective, the crista is centered between the paired orbital plates of the frontal bone (if included) and sits immediately anterior to the cribriform plate, whose perforations for olfactory fila lie laterally on either side. Its anterior margin blends with the frontoethmoidal region while its posterior surface faces the anterior cranial fossa, forming a palpable midline landmark in the skull base. Midline anatomy, clearly organized. Attachment of the falx cerebri to the crista galli makes this small projection clinically loud: traction, tumor, or surgical manipulation at this anchor point transmits directly to dural reflections and the superior sagittal sinus region. Fractures crossing the adjacent cribriform plate, often from frontal trauma, can tear meningeal coverings and produce cerebrospinal fluid rhinorrhea with associated anosmia from injury to olfactory nerve filaments traversing the ethmoidal roof. A focused anterior view also helps clarify why endoscopic endonasal approaches treat the crista galli as a caution zone, since the thin ethmoid roof lateral to the midline is vulnerable to iatrogenic skull base violation during ethmoidectomy or frontal sinus work. Neuroanatomy and head and neck anatomy courses use this illustration to teach anterior cranial fossa landmarks, dural attachments, and the relationship of the ethmoid complex to the nasal cavity and paranasal sinuses.
