Description
Crista galli rises as a midline ethmoidal crest from the superior surface of the cribriform plate, centered between the right and left olfactory fossae. From this superior viewpoint, the crista stands anterior to the planum sphenoidale and just posterior to the frontal bone’s ethmoidal notch, with the perforated cribriform plate spreading laterally on either side. Multiple foramina punctuate the cribriform plate for olfactory nerve filaments (CN I), and the crista galli forms the medial buttress that divides these perforations into right and left fields. Midline anatomy is unambiguous. Neurosurgeons and ENT surgeons care about the crista galli because it anchors the falx cerebri at the anterior cranial fossa, and its relationship to the cribriform plate defines the boundary where dural reflections and olfactory fibers sit adjacent to the nasal cavity. Fracture through the cribriform plate near the crista galli can disrupt olfactory fila and create a direct route for cerebrospinal fluid rhinorrhea, a classic anterior skull base injury pattern after facial trauma. The superior perspective also helps clarify why aggressive instrumentation at the skull base during endoscopic sinus surgery risks breaching the anterior cranial fossa, especially when the cribriform plate is thin or asymmetric around the midline crest.
