Description

Medially, the renal hilum interrupts the smooth anterior contour of the kidney as a vertical cleft leading into the renal sinus. From an anterior perspective, the renal vein lies most anterior at the hilum, the renal artery typically sits posterior to it, and the renal pelvis continues inferiorly as the proximal ureter. Segmental branches and tributaries cluster at this indentation, framed by the anterior and posterior lips of the renal parenchyma and the perihilar fat within the sinus. An anterior hilum view matters because it matches the operative corridor for open and laparoscopic exposure when the colon is reflected and the renal pedicle is approached from the medial side. Order matters. Remembering the usual anterior-to-posterior relationship (vein, artery, pelvis) reduces pedicle confusion during hilar dissection, vascular control, or living donor nephrectomy, and it also supports interpretation of CT urography when assessing ureteropelvic junction obstruction, renal artery stenosis, or tumor extension into the renal vein.
