Aggregated erythrocytes (red blood cells) form the dominant cellular mass, packed tightly within an interlacing fibrin mesh that spans the clot from one platelet-rich focus to the next. Platelets appear as irregular, stellate elements adherent to fibrin strands and clustered along the developing thrombus surface, where they would lie closest to the injured endothelium in vivo. Scattered leukocytes, larger and paler than surrounding erythrocytes, sit interspersed within the fibrin network and between red cell stacks, reflecting how inflammatory cells become incorporated as the clot organizes. Thrombus architecture matters because it determines both stability and downstream risk. Platelet-dominant, fibrin-poor thrombi form under high shear (classically arterial) and drive acute coronary syndromes and ischemic stroke, while erythrocyte-rich fibrin thrombi predominate in low-flow venous beds and underlie deep vein thrombosis and pulmonary embolism. This composition also predicts treatment response: antiplatelet agents target platelet adhesion and activation, whereas anticoagulants blunt thrombin generation and fibrin formation, and fibrinolytics act on the fibrin scaffold that traps the cellular elements.
