Description
Anterior sectional anatomy through the upper abdomen reveals the C-shaped duodenum wrapping around the head of the pancreas, with the duodenal lumen encircled by mucosa and muscularis and the pancreatic parenchyma sitting posteromedial to the descending part. The superior (first) part transitions into the descending (second) part to the patient’s right, while the horizontal (third) part courses leftward inferior to the pancreatic head. Along the medial wall of the second part, the hepatopancreatic ampulla region is implied, where the common bile duct and main pancreatic duct typically converge before opening at the major duodenal papilla. This plane matters when you need to teach, or plan, how periampullary disease produces both obstructive jaundice and pancreatitis: a small lesion at the ampulla can impede bile flow and pancreatic drainage at the same choke point. Surgeons and endoscopists rely on these relationships during ERCP and sphincterotomy, and the same anatomy defines the exposure and dissection planes in pancreaticoduodenectomy (Whipple procedure), where the duodenum, pancreatic head, and distal bile duct are removed as a single specimen. It also clarifies why edema or hematoma in the duodenal wall can compress adjacent ductal structures in blunt trauma.
