Description
Sectioned through the kidney, the renal sinus appears as a centrally placed cavity that opens medially at the hilum and extends laterally into the renal parenchyma. Within this space, the funnel-shaped renal pelvis sits centrally and continues distally as the proximal ureter, while major calyces branch into minor calyces that cup the renal papillae. Renal sinus fat occupies the intervening recesses and wraps the collecting system and neurovascular elements, with the renal artery and vein positioned anteroposteriorly at the hilum in the usual vein, artery, pelvis sequence from anterior to posterior. A cross-sectional look at the renal sinus matters when you need to distinguish true intrarenal collecting system dilatation from look-alikes. Hydronephrosis expands the renal pelvis and calyces within the sinus and can be followed to a point of ureteropelvic junction obstruction, while parapelvic cysts arise from sinus structures and may compress the pelvis without communicating with it.
