Description
Entering the thorax from the midline, the trachea bifurcates at the carina into the right and left primary bronchi, shown in anterior view as they pass inferolaterally toward their respective hila. The right main bronchus runs shorter, wider, and more vertical than the left, a relationship that stays apparent even in a frontal overview. At each hilum, the primary bronchi begin their branching into lobar (secondary) and segmental (tertiary) bronchi, spreading within the pulmonary parenchyma in a tapering tracheobronchial tree. Proximally, the airway caliber is largest near the carina and narrows distally into smaller bronchial passages. Anterior depictions of the primary bronchi matter because this is the orientation most clinicians carry into bronchoscopy, endotracheal intubation checks, and plain chest radiograph interpretation. A right-sided mainstem intubation tracks the anatomy, the right primary bronchus is the common unintended destination, and you can tie that directly to unilateral breath sounds and ipsilateral hyperinflation. Aspiration follows the same geometry, with foreign bodies and gastric contents preferentially lodging in the right bronchial system, often the bronchus intermedius or basal segmental bronchi, depending on body position.
