Description
Anterior orientation of the lungs places the left main bronchus as it courses inferolaterally from the tracheal bifurcation, giving rise to the left lobar bronchi and then segmental bronchi within the inferior lobe. The basal segmental bronchus is traced distally into the lower lobe parenchyma, running inferior to the superior segmental bronchus and distributing toward the basal segments along the diaphragmatic surface. Medially, its proximal portion lies near the pulmonary hilum, while its distal branches spread laterally and inferiorly toward the costophrenic recess. Branching matters. Segmental bronchial anatomy underpins both interpretation of thoracic CT and the logic of anatomic lung resection. On the left, surgeons and bronchoscopists rely on the relationship between segmental bronchi and segmental pulmonary arteries when planning a left lower lobe segmentectomy, and the basal segmental bronchus becomes a key landmark when targeting endobronchial interventions or assessing postobstructive atelectasis in the dependent lung base. Mucus plugging or an aspirated foreign body often settles distally, and basal segment involvement can help explain a recurrent lower lobe opacity on the left.
