Description
Superiorly oriented profile of the left bronchial tree centers on the left inferior lobar (lower lobe) bronchus and its segmental branches, with emphasis on the anterior basal segmental bronchus (bronchus segmentalis anterobasalis sinister, B8). From this overhead perspective, the B8 bronchus arises from the basal bronchial trunk of the lower lobe and courses anteroinferiorly toward the anterior basal segment of the left lower lobe. Neighboring basal segmental bronchi to the lateral and posterior aspects provide the expected segmental context even when only their proximal origins are included. Clear identification of the separate origin and trajectory of B8 matters in bronchoscopy and in preoperative planning for anatomic segmentectomy of the left lower lobe, where a surgeon must follow the segmental bronchovascular pedicle without violating adjacent segments. Segmental bronchi are also used as landmarks when correlating endobronchial anatomy with CT bronchography, because mucus plugging, post-obstructive atelectasis, or an endobronchial lesion confined to B8 will produce a segmentally distributed pattern in the anterior basal parenchyma. Orientation from above helps you keep the anterior and posterior basal pathways straight. Small airway, big consequences.
