Description
Arising from the anterior division of the internal iliac artery, the uterine artery courses medially within the base of the broad ligament and reaches the uterus at the level of the cervix, where it ascends tortuously along the lateral uterine wall toward the fundus. Branches perforate the myometrium to form arcuate arteries that run circumferentially, giving rise to radial arteries that pass inward to the endometrium as straight and spiral arteries; accompanying uterine veins track alongside these arterial channels and drain to the internal iliac venous system. Near the uterine cornu, the tubal and ovarian branches anastomose with the ovarian artery along the mesosalpinx and utero-ovarian ligament. Dense, layered flow. That ascending uterine artery, with its characteristic tortuosity, is the operative landmark for uterine-sparing hemostasis and hysterectomy. The relationship of the ureter passing inferior to the uterine artery near the cervix (water under the bridge) underlies one of the most common avoidable injuries during ligation and cautery. Endometrial perfusion through the spiral arteries also explains the clinical pattern of ischemic shedding in menstruation and the target of embolization in postpartum hemorrhage and symptomatic fibroids. Use this profile for teaching pelvic blood supply in gross anatomy and OB-GYN modules, for illustrating uterine artery embolization in interventional radiology content, or for labeling exercises that contrast myometrial arcuate vessels with endometrial spiral arteries in reproductive physiology texts.
