Seen from a lateral periphery, the left ventricular apex occupies the inferior and anterior portion of the cardiac silhouette, tapering to the tip where fine apical trabeculations form a delicate mesh along the endocardial surface. More superiorly the ventricular cavity widens, and the trabecular ridges coalesce into thicker muscular bands that grade toward the smoother mid-ventricular wall. Anteriorly, the apical myocardium lies just deep to the left costal margin, while posteriorly it approaches the diaphragmatic surface; the trabeculae themselves project into the lumen rather than outward toward the epicardium. Fine trabeculation at the left ventricular apex matters because it sits at the intersection of normal anatomic variation and imaging overcall.
