Description
Hook of the hamate (hamulus ossis hamati) projects palmarly from the ulnar side of the distal carpal row, arising from the anterior surface of the hamate body and curving toward the hypothenar eminence. From this anterior (palmar) viewpoint, the hamulus sits distal to the triquetrum and pisiform region and ulnar to the capitate, forming a prominent bony spur that borders the ulnar aspect of the carpal tunnel. The base of the hook appears broad where it merges with the hamate, while the tip narrows and points anteriorly and slightly radially. A small bone with big consequences. Clinically, the hamulus is the palpable and radiographic landmark implicated in hook of hamate fractures, classically seen in golfers, racquet sport athletes, and baseball batters when a handle transmits force into the hypothenar side of the palm. Its position as the radial wall of Guyon’s canal and an attachment site for the flexor retinaculum and pisohamate ligament explains why injuries can produce ulnar nerve sensory changes in the ring and little fingers, intrinsic hand weakness, or ulnar artery irritation, and why chronic nonunion may present as pain during grip with flexor tendon irritation.
