Description
Suspended among biconcave erythrocytes, a spherical hepatitis B virus (HBV) virion appears as an enveloped particle with prominent surface projections consistent with hepatitis B surface antigen (HBsAg) embedded in a lipid membrane. Beneath the envelope, the implied icosahedral nucleocapsid (HBcAg) encloses the relaxed circular, partially double stranded DNA genome typical of the Dane particle. Red blood cells occupy the surrounding field as passive bystanders, creating a true intravascular context, while the virion sits in the plasma space between cells rather than attaching to formed elements. Viremia is the bridge between exposure and hepatic infection, and this blood centered view aligns with how HBV is encountered in real clinical workflows: needlestick injuries, hemodialysis circuits, transfusion screening, and perinatal transmission risk assessment. A single circulating particle also cues an often missed teaching point, most of what circulates during active infection is not intact virion but excess HBsAg subviral spheres and filaments, which drives serologic positivity even when infectivity changes. Small scale, big consequence.
