Description
Cutaneous cross-section centers on a large cystic acne nodule seated in the deep dermis, with a dilated pilosebaceous unit as the point of origin. Superiorly, the stratified epidermis overlies a thick dermis containing adnexal structures, while inferiorly the process approaches the subcutaneous adipose layer. Laterally, adjacent hair follicles, sebaceous glands, eccrine coils, dermal nerves, and a superficial and deep vascular plexus frame the lesion, helping orient depth and potential spread. Inflammatory edema expands the dermis around the cyst cavity. Rupture or near-rupture of the follicular wall is the teaching hinge in cystic acne, because keratin, sebum, and Cutibacterium acnes antigens escape into the dermis and drive a foreign body granulomatous response with neutrophils, macrophages, and later fibrosis. That depth explains the tenderness and the postinflammatory sequelae clinicians see, including atrophic or hypertrophic scarring, sinus tract formation in acne conglobata, and the limited effect of superficial therapies that do not reach the lower follicular segment. It also maps cleanly to procedure planning: intralesional triamcinolone targets the inflammatory nodule in the mid to deep dermis, while incision and drainage risks tracting and scarring if performed through actively inflamed tissue.
