Description
Centered on the skin surface is a small, circumscribed pustule, an elevated lesion with a superficial collection of purulent material within the epidermis and upper dermis. Beneath it, the epidermis overlies a vascular dermis containing coiled eccrine sweat glands, arterioles and venules, and fine cutaneous nerve fibers, with hair follicles and their associated sebaceous glands positioned more deeply and obliquely. Adipose lobules in the hypodermis form the inferior boundary of the section. Orientation stays true to anatomical position: superficial epidermis, deep subcutis, and adnexal structures coursing from deep to superficial toward the pore and follicular ostium. Pustules are a core teaching lesion in dermatology because they force you to distinguish follicular from nonfollicular inflammation, and to separate acneiform disease from infectious pustulosis. In acne vulgaris, neutrophil-rich inflammation often tracks around the pilosebaceous unit, with obstruction at the follicular infundibulum and rupture driving a perifollicular dermal response that correlates with tenderness and post-inflammatory hyperpigmentation. Small differences in depth matter. A more superficial pustule behaves differently from a deeper follicular abscess when you plan drainage, culture, or topical versus systemic therapy.
