Description
Axis (C2) inferior articular facet occupies the inferolateral margin of the vertebra, forming the superior half of the C2 to C3 zygapophysial (facet) joint. From an anterior viewpoint, the inferior articular process sits posterior to the vertebral body and dens, with the facet surface turning inferiorly and slightly laterally toward the superior articular facet of C3. Medial to the facet, the pedicle and the pars interarticularis create a stout bony bridge between the superior and inferior articular processes, while the lamina and spinous process lie further posterior and are only partly appreciated from this fixed perspective. Orientation matters here. Facet anatomy at C2 is a daily reference point in cervical trauma and stabilization, because fractures through the pars interarticularis (classic hangman’s fracture) separate the posterior elements from the vertebral body and can destabilize the C2 to C3 segment while sparing the spinal canal early on. The anterior view helps you relate the inferior articular facet to the vertebral body, pedicle, and pars, a relationship that guides safe C2 pedicle or pars screw trajectories and explains why malreduction can leave residual facet malalignment and pain. Surface curvature and margins are easy to read, which supports teaching joint congruency and interpreting CT correlates.
