Axis (C2) laminae form the paired bony plates that complete the posterior vertebral arch, meeting in the midline to create the base of the bifid spinous process. From a posterior standpoint, each lamina extends medially from its junction with the pedicle and pars interarticularis, curving toward the midline just posterior to the vertebral foramen and spinal canal. Superiorly, the laminae relate to the superior articular processes that articulate with the atlas (C1), while inferiorly they frame the inferior articular processes for the C2 to C3 zygapophysial joints. Cortical margins and trabecular thickness are emphasized where the lamina contributes to the posterior boundary of the canal. Solid bone. C2 laminar anatomy matters in trauma and posterior cervical instrumentation because it defines the safe corridors around the canal and the vertebral artery, which courses laterally within the transverse foramen rather than through the lamina itself. In hyperextension injuries and high-energy motor vehicle trauma, fractures involving the C2 posterior elements may accompany pars injuries (the so-called hangman’s fracture pattern), so distinguishing lamina from pedicle and pars is not academic, it changes fixation strategy. The fixed posterior perspective also supports teaching of ligamentous attachments, including the ligamentum flavum spanning adjacent laminae and the nuchal ligament attaching along the spinous process, landmarks that guide midline dissection in a posterior approach. Spine anatomy labs, neurosurgical and orthopedic teaching files, and radiology texts pairing 3D osseous anatomy with CT bone windows commonly need an isolated, correctly proportioned axis to annotate laminar borders, midline fusion, and the relationship of lamina to the zygapophysial joints.
