Anteriorly on the visceral (inferior) surface of the liver, the medial segment of the left hepatic lobe (Couinaud segment IV, the quadrate lobe in older terminology) occupies a wedge between the gallbladder fossa laterally and the fissure for the ligamentum teres (round ligament) medially. Posterior to segment IV, the porta hepatis forms a transverse groove where the portal vein lies posterior to the proper hepatic artery and common hepatic duct before they enter the liver as the portal triad. Inferiorly and posteriorly, the caudate lobe (segment I) sits adjacent to the fissure for the ligamentum venosum and the inferior vena cava. Orientation of segment IV on the inferior surface matters because it is a common reference point for hepatobiliary surgery and interventional radiology: it bridges the functional left and right livers and lies immediately superior to the gallbladder and cystic plate, where inflammation or adhesions can obscure planes. Segment IV is frequently traversed during left hepatectomy extensions, cholecystectomy-related dissection near the gallbladder bed, and intraoperative ultrasound mapping of portal pedicles, and its proximity to the middle hepatic vein territory explains why venous bleeding can occur when parenchymal transection strays too close to the midplane. Small landmarks drive big decisions. Faculty often need this inferior perspective when teaching Couinaud segmentation, the porta hepatis, and peritoneal reflections in gross anatomy or surgical anatomy blocks, because learners mix up the quadrate lobe label with true functional segmental anatomy.
