Description
Posteriorly oriented anatomy centers on the liver’s posterior medial segment (Couinaud segment VIII), positioned in the superior right hepatic lobe just inferior to the diaphragm and anterosuperior to the porta hepatis. Medially, the segment abuts the middle hepatic plane near the gallbladder fossa anteriorly, while its posterior surface approaches the bare area where liver contacts diaphragm without peritoneum. Lateral continuity with the anterior sector of the right lobe is implied, and the caudate lobe (lobus caudatus, including the Spigelian lobe) lies posteromedially, draped between the inferior vena cava groove and the ligamentous fissures. Short hepatic veins drain from the dorsal surface toward the inferior vena cava. A tight neighborhood. Surgeons and interventional radiologists often refer to segment VIII when planning right hepatectomy, anterior sectionectomy, or segmental resection for hepatocellular carcinoma or colorectal liver metastasis, because its deep superior position can obscure margins and complicate inflow control. This posterior perspective helps orient the relationship between the dorsal liver, the caudate process, and the retrohepatic inferior vena cava, a relationship that matters during mobilization of the right lobe and control of bleeding from short hepatic veins. Segment VIII also sits close to the hepatic venous confluence, so venous outflow anatomy drives safe transection planes. Ideal use cases include hepatobiliary surgery atlases, radiology teaching files correlating Couinaud segments to CT and MRI, and gross anatomy or abdominal surgery courses that need clear posterior landmarks for segmental liver anatomy in an adult male. Licensing fits journal figures on liver resection planning, transplant anatomy discussions, and clinical education on retrohepatic vena cava exposure.
