Description
Caudate lobe (lobus caudatus, Spigelian lobe) is presented as a posterior, superior segment of the male liver, tucked between the inferior vena cava posteriorly and the fissure for the ligamentum venosum on its left. Medially it hugs the caval groove, while laterally it blends toward the right hepatic lobe and the hepatic venous confluence. Superiorly the parenchyma approaches the bare area of the liver, with the inferior margin continuing toward the porta hepatis via the caudate process. Surgeons and radiologists care about the caudate lobe because it behaves like its own unit. Portal inflow and biliary drainage can be relatively independent, and venous outflow commonly drains directly into the inferior vena cava through short hepatic veins rather than through the main right, middle, and left hepatic veins. That anatomy explains the classic pattern in Budd-Chiari syndrome, where hepatic venous outflow obstruction can lead to caudate hypertrophy and secondary compression of the inferior vena cava. It is also the segment that challenges hepatic resections, because bleeding from short hepatic veins occurs close to the caval wall.
