Description
Seen from the inferior surface of the male liver, the caudate lobe (lobus caudatus, Spigelian lobe) occupies the posterior aspect of the visceral surface, superior to the porta hepatis and medial to the groove for the inferior vena cava. The fissure for the ligamentum venosum runs along its left margin, separating it from the left lobe, while the caudate process extends inferiorly toward the right lobe and the region of the hepatic hilum. Posterior relations dominate. A small territory with outsized landmarks. An inferior view is where the caudate lobe makes anatomic sense in operative and cross sectional correlation, because it is the only hepatic segment (Couinaud segment I) with direct venous drainage to the inferior vena cava via short hepatic veins and with portal inflow that can arise from both right and left portal branches. That arrangement explains why caudate hypertrophy is a familiar accompaniment of cirrhosis and Budd Chiari syndrome, and why isolated caudate resection demands close attention to the retrohepatic IVC and the ligamentum venosum fissure as a dissection plane. Surgeons and radiologists both use these inferior surface landmarks to separate hilar structures from the posterior segmental anatomy.
