Viewed from an inferior, caudal perspective, the paired lungs are oriented around the thoracic vertebral column, with emphasis on the vertebral surface (facies mediastinalis, pars vertebralis) where each lung closely apposes the spine. Medially, the colored vertebral surfaces form a longitudinal contact zone adjacent to the posterior mediastinum, while laterally they transition into the costal surface (facies costalis) that conforms to the inner curvature of the ribs. Posterior margins track alongside the vertebral bodies, and the inferior borders sweep anterolaterally to frame the bases. For teaching, this angle clarifies how the posterior lung relates to deep thoracic landmarks that matter in exam and procedure. Posterior basal segments (S10) sit low and dorsal, explaining why aspiration pneumonia and dependent atelectasis often localize to the lower lobes in supine patients and why posterior auscultation targets the bases near the costovertebral angles.
