Description
Oriented in a medial perspective, the left lung is presented with its mediastinal (medial) surface facing the viewer, so the cardiac impression occupies the anteroinferior portion while the posterior part of the lung lies close to the thoracic vertebral column. Superiorly, the apex rises above the level of the first rib, and inferiorly the diaphragmatic surface curves into the base, where colored posterior and posterobasal segmental territories are emphasized. Along the hilum, the relationship of the pulmonary root is implied, with the main bronchus positioned posteriorly, the pulmonary artery superior to the superior pulmonary vein, and the descending thoracic aorta running posterior to the root to create a long, vertical groove. Left-sided asymmetry is clear. Medial surface anatomy matters when you need to teach why the left lung molds to the heart and great vessels and why posterior basal segments sit deep against the vertebral bodies. This angle supports clinical reasoning in thoracic imaging: dependent consolidation or atelectasis in the left lower lobe often involves the posterior basal segment on supine CT, and effusions first blunt the posterior costodiaphragmatic recess rather than altering the anterior margin. Surgeons also work in this corridor. Understanding the mediastinal pleura and the posterior relationship to the descending aorta helps when planning left lower lobectomy, segmentectomy, or mediastinal lymph node dissection.
