Description
Medial (mediastinal) surfaces of the right and left lungs are presented in posterior view, emphasizing the visceral pleura and the topography of adjacent mediastinal organs. On the left lung, the cardiac impression occupies the anteroinferior medial surface, with the grooves for the aortic arch and descending thoracic aorta tracking superiorly then posteriorly along the lung’s medial border. On the right lung, a deeper impression for the right atrium lies anteriorly, while vertical sulci for the superior vena cava and the arch of the azygos vein sit more superomedial, and the esophageal impression runs posterior to the hilum. Hilar regions are implied by the surrounding contours where bronchi and pulmonary vessels would enter and leave the lung. Posterior emphasis on the mediastinal faces is where relationships that matter in thoracic imaging and operative planning become easier to teach, because many clinically relevant contacts are posterior or posteromedial. A left-sided descending aortic aneurysm can distort the aortic groove and compress adjacent lung, and subcarinal or paraesophageal lymphadenopathy can efface the esophageal impression and narrow bronchial lumina, patterns radiologists often correlate on CT. For surgeons, the right mediastinal surface landmarks help orient during posterolateral thoracotomy or VATS when mobilizing the lung near the azygos arch and superior vena cava. Orientation matters. Use this asset in gross anatomy labs to reinforce mediastinal relations, in radiology teaching files to pair with axial CT through the hila, and in cardiothoracic surgery materials discussing approaches near the pulmonary hilum and posterior mediastinum.
