Description
Arising distal to the right superior lobar bronchus, the right intermediate bronchus is seen from an inferior perspective as a relatively long airway segment descending toward the hilum before giving off the middle lobar bronchus anterolaterally and continuing into the lower lobar bronchus more inferiorly. From below, the lumen sits medial to the right lung parenchyma and lateral to the mediastinal side of the hilum, with the branching points separating middle lobe supply from lower lobe supply along a single axial conduit. Segmental bronchi to the lower lobe originate distal to the lower lobar takeoff, while the middle lobe segmental branches arise proximal to that level, emphasizing the intermediate bronchus as the key divider between these lobar origins. Clean branching anatomy. This angle matters when teaching right-sided airway organization because the intermediate bronchus is the corridor where bronchoscopy orientation can drift between the middle and lower lobes, and small changes in scope tip position can select the wrong lobar entrance. In obstructive disease, mucus plugging or an aspirated foreign body lodged in the intermediate bronchus can produce combined middle and lower lobe atelectasis, a pattern that helps distinguish it from isolated lower lobe collapse.
