Seen from an inferior perspective, the right lower lobe bronchial tree is centered on the basal segmental bronchus (bronchus basalis), with its segmental branches radiating distally toward the diaphragmatic surface of the lung. The bronchus to the superior segment of the lower lobe lies more posterior and does not dominate this view, while the basal segmental bronchi diverge anterolaterally and posteromedially to supply the basal segments. Expect clear separation of the segmental origins for the medial basal (S7), anterior basal (S8), lateral basal (S9), and posterior basal (S10) territories, arranged around the bronchial axis in a pattern that mirrors the segmental anatomy of the right lower lobe. An inferior view of the basal segmental bronchus matters because segmental bronchi define surgical and bronchoscopic roadmaps in the lower lobe, where orientation is often lost once you move beyond the lobar bronchus. Precise identification of S7 to S10 supports segment-directed bronchoscopy, targeted sampling of suspected peripheral lesions, and planning of an anatomic segmentectomy when lobectomy is undesirable. Misidentifying the posterior basal bronchus (B10) versus the lateral basal bronchus (B9) is a common pitfall during navigation for biopsy and during resection along intersegmental planes. Orientation is everything.
