Description
Arising from the bronchus intermedius, the right lower lobar bronchus descends inferolaterally and gives off the basal segmental bronchus, seen here from a posterior aspect within the lower lobe. The basal segmental bronchus courses toward the posteroinferior lung, positioned inferior to the superior (apical) segmental bronchus of the right lower lobe and lateral to the medial basal territory near the hilum. Branching is shown as a stout, airway-caliber structure relative to adjacent segmental bronchi, consistent with its distribution to the basal segments of the right lower lobe. Posterior orientation matters because it mirrors how the right lower lobe is approached and assessed in thoracic imaging and bronchoscopy when targeting dependent basal segments. This is the drainage and aspiration pathway. In right-sided aspiration pneumonia and basal atelectasis, secretions commonly track into lower lobe basal segmental bronchi, and bronchoscopic toilet or lavage often targets these segmental or subsegmental takeoffs under direct visualization.
