Description
Posteriorly, the right lower lobe bronchial tree is opened to emphasize the posterior basal segmental bronchus (B10) as it descends into the posterior basal segment (S10) near the diaphragmatic surface. Superior to it, the superior segmental bronchus (B6) typically courses toward the apical portion of the lower lobe, while the lateral and anterior basal branches (B9 and B8) angle more laterally and anteriorly from the inferior lobar bronchus. Medially, the bronchial origin sits closer to the hilum and pulmonary ligament, and distally the airway tapers toward the segmental and subsegmental bronchi embedded within the posterior basal parenchyma. Posterior basal anatomy matters because B10 is a common pathway for dependent aspiration and postural secretion pooling in the supine patient, with recurrent infection or atelectasis often localizing to S10. For bronchoscopy, this branch can be deceptively straight from the inferior lobar bronchus, and understanding its posterior trajectory helps when directing the scope and correlating endobronchial findings with CT bronchial mapping. Segmental orientation also guides limited resections, since a posterior basal segmentectomy requires confident identification of B10 and its relationship to neighboring basal branches and the inferior pulmonary vein tributaries that border the intersegmental plane.
