Description
Posteriorly, the right main bronchus continues as the bronchus intermedius and gives rise to the right lower lobar (inferior) bronchus, from which the medial basal segmental bronchus (B7) originates low and medially within the basal pyramid. From this back-facing perspective, B7 tracks anteromedially relative to the posterior basal segmental bronchus (B10) and inferior to the superior segmental bronchus (B6), with the basal segmental bronchi fanning distally toward their respective bronchopulmonary segments. The right lower lobe occupies the inferoposterior field, with the segmental branching pattern read from proximal to distal along the bronchial tree. Small, deep airways. Big implications. Bronchoscopists and thoracic surgeons care about B7 because it is an anatomic outlier in the right lower lobe, often arising close to the takeoff of neighboring basal segmental bronchi and angling medially toward the cardiomediastinal surface.
