Description
Arising from the carina, the left main (primary, chief) bronchus courses inferolaterally from the trachea toward the left lung hilum. From a left lateral orientation, its posterior relationship to the left atrium and its passage inferior to the aortic arch are emphasized, with the pulmonary trunk lying anterior and superior as it crosses toward the left pulmonary artery. Near the hilum, the airway divides into the left superior lobar bronchus and left inferior lobar bronchus, the bifurcation occurring distal to the arch and proximal to segmental branching. Short and direct. That leftward angle matters. The left main bronchus is longer and more horizontal than the right, and its confinement beneath the aortic arch helps explain why left-sided endobronchial intubation is less common yet more difficult to correct when it occurs. This corridor is also where extrinsic compression becomes clinically legible, classically from a dilated left atrium in mitral valve disease or from aortic arch aneurysm, producing cough, wheeze, or recurrent left lower lobe atelectasis that can mimic asthma.
