Description
Arising from the bronchus intermedius, the right middle lobar bronchus courses anterolaterally from its posterior origin, positioned inferior to the right superior lobar bronchus and superior to the right inferior lobar bronchus. From the rear, the intermediate airway sits slightly right of midline, with the middle lobar takeoff appearing as a short, stout branch directed toward the middle lobe between the oblique and horizontal fissure regions. Proximally, the lumen is continuous with the primary (main) bronchus via the intermediate segment, and distally it leads toward the segmental bronchi of the middle lobe. A posterior perspective on the right bronchial tree is the one you want when teaching why the right middle lobe is a frequent site of atelectasis and recurrent infection. The middle lobar bronchus is relatively narrow and takes off at an angle that can impair drainage, so edema, mucus plugging, or extrinsic compression by peribronchial lymph nodes can precipitate right middle lobe syndrome. Bronchoscopists and thoracic surgeons also use these relationships to confirm they are distal to the right upper lobe bronchus but proximal to the basilar segments before sampling, suctioning, or planning a middle lobectomy. Orientation matters.
