Description
Viewed from a posterior, slightly superior perspective, the right and left main (primary, stem) bronchi are presented at the level of the carina as they diverge laterally from the distal trachea, each maintaining a wide lumen supported by C shaped cartilaginous rings. Posteriorly, the bronchial wall transitions to the membranous portion where the trachealis and bronchialis smooth muscle spans between cartilage ends, a key distinction from the rigid anterior and lateral arcs. The right main bronchus typically courses more vertically and with a larger caliber than the left, which angles inferolaterally beneath the aortic arch toward the left hilum. Posterior bronchial anatomy matters when you are thinking about airway instrumentation and the consequences of bronchial asymmetry. Right sided endobronchial intubation remains a common malposition because the right main bronchus is shorter, wider, and more in line with the trachea, and this view helps explain why the left lung can be silently under ventilated.
